Lampard scored 211 goals in 648 appearances for Chelsea, helping them to three Premier League titles, four FA Cup triumphs and two successes in the League Cup.
He was also a member of the Blues team who won the Champions League for the first time in their history in 2012, as well as lifting the Europa League crown the following season.
Lampard netted 29 times in 106 appearances for England, while he also played club football for West Ham, Manchester City and New York City.
Lampard told the club's official website after being confirmed in his new role: "I am immensely proud to be returning to Chelsea as head coach.
"Everyone knows my love for this club and the history we have shared, however, my sole focus is on the job in hand and preparing for the season ahead.
"I am here to work hard, bring further success to the club and I cannot wait to get started."
Blues director Marina Granovskaia added: "Frank possesses fantastic knowledge and understanding of the club and last season he demonstrated he is one of the most talented young coaches in the game.
"We will do everything we can to ensure he has all the support required to be a huge success."
Chelsea can be backed at 25/1 to win the Premier League title next season, while they are 5/4 to finish in the top four.
